1662년 파리, 루이왕(레오나르도 디카프리오)은 굶주린 백성들은 외면한 채 전쟁과 향락에 취해 살았다. 은퇴한 삼총사는 각자의 삶을 살고 있었다. 포르토스(제라르 드파르디유)는 여자와 즐기면서, 아토스(존 말코비치)는 아들 라울을 희망 삼아서, 아라미스(제레미 아이언스)는 신부로서의 삶을 살고 있었다. 달타냥(가브리엘 번)은 왕의 경호대장으로서 삼총사와는 달리 바쁘게 살아가고 있었다. 아토스의 아들 라울은 아버지의 뜻에 따라 명예로운 기사가 되고, 사랑하는 여인 크리스틴에게 청혼한다. 그러나 루이왕이 점 찍은 크리스틴과의 결혼은 순탄치 않다. 결국 루이왕은 라울을 전쟁터로 보내고 크리스틴을 차지한다. 크리스틴은 죄책감에 시달리다 자살하고, 아들을 잃은 아토스는 복수를 결심한다. 사실 루이왕은 쌍둥이로 태어났는데, 한쪽은 아이언 마스크를 쓴 채 갇혀 있었다. 이 사실을 안 삼총사는 이들 형제를 바꿔치기할 계획을 세우는데...
