Tae Soo devises a plan and saves Yoon Seo, the girl who desperately needs a new heart. With the help of the surgical team and by duping Director Yoon, Soo Yeon secretly operates on Yoon Seo. Meanwhile, with all eyes on Taesan University Hospital, Director Yoon orders Professor Choi to announce to the press that Han Min Sik's heart transplant was a success. Yoon Seo recovers fully with her new heart, but with no heart to give Han Min Sik, the situation gets dire for Director Yoon. Stealing one is his only option. Later, with a change of heart, Professor Choi turns the table and puts Director Yoon in the light with unequivocal evidence.

The atmosphere in the hospital is tense, and everyone is feeling Director Yoon's wrath as a patient dies due to his bias for the wealthy. Director Yoon tells Professor Choi that Taesan will be setting up a cardiothoracic hospital in Russia and warns him that there mustn't be any complications with Han Min Sik's heart transplant. Meanwhile, Tae Soo and Soo Yeon worry that Director Yoon is up to no good. Director Yoon notices a newly listed child for a heart transplant with both her blood and tissue identical to Han Min Sik's. Later, a code blue occurs and the child, Yoon Seo, goes into cardiac arrest. If it weren't for Soo Yeon and Tae Soo, Director Yoon would have gotten his way and had her die. Luckily, Yoon Seo makes it onto the ECMO and jumps up in priority for a heart, putting Director Yoon in a predicament.

The operating team leaves Professor Choi's surgery and joins Tae Soo to save an emergency patient. For his direct disobedience, Director Yoon orders to fire Tae Soo, but Professor Choi changes his mind. Director Yoon finds out that there are too many patients staying longer than needed and orders Chief Koo of the Cardiothoracic Department to weed out the unneeded patients. Acting on her suspicion, Soo Yeon meets with the daughter of Han Min Sik's aide, who died on the operating table, and digs for answers. Meanwhile, Han Min Sik confirms his candidacy for the presidential election, and Professor Choi asks Director Yoon for something more than just his word.

When Han Min Sik's aide dies on the operating table, Director Yoon smiles. However, Soo Yeon, who is watching the surgery with him, notices his smile. Meanwhile, Professor Lee and Chief Koo ignorantly think that their chances for the chiefship is between themselves and start to giggle at Professor Choi's supposed downfall.
